How does “Option 82 insert only” mode work if an AP sees a DHCP request that already has an Option 82 setting? I ask because we have an “Option 82 insert only” AP that will be feeding a small site where we would like to host another Option 82 insert only AP.

Yep. Full relay is the only way to make it work currently.
