1) The disconnect reason is in the logs you posted. The device is not able to reach cnMaestro and therefore Device Agent disconnects from it. Can you try a ping test from the device to the url of cnMaestro to double check if it can be reached? Also, make sure DNS server is reachable as well. Remember that both port 443 and 80 must be open in your network for cnMaestro to work.
2) If the Device Agent is not able to reach cnMaestro, it is possible that it will show as offline on cnMaestro while in reality all other radio functions are operating normally.
